Will my health insurance pay for my sleep study?
Although most insurance plans pay for sleep disorders medicine services (consults, testing and follow-up care), the amount and type of coverage depends on your particular policy and plan. Please make sure that you check with your insurance carrier to determine the payments that they will make for sleep medicine services.
What will my out-of-pocket expenses be?
Out-of-pocket expenses will vary with insurance plans and can be affected by co-pays and the amount of deductible that you may or may not have met. It is important, and your responsibility, to check with your insurance carrier about coverage for specific services and testing as well as what co-pays and deductibles will be your responsibility.
Do I need a pre-approval for sleep medicine services?
Please contact your insurance carrier directly to obtain information concerning prior authorization requirements. Sleep Medicine Associates, PC can assist you in gathering information that you might need to obtain any pre-approvals that your insurance carrier might require prior to services being provided.
About Paying My Bill

When will I be expected to pay for consultation and testing services?
Payment is expected at the time services are rendered. Any co-payments that you are responsible for under your insurance policy or plan is due at the time services are rendered. If claims are submitted to insurance carriers for covered services, you will receive a bill for the balance that remains after payment by your insurance carrier has been made. This balance is considered payable upon receipt, unless other arrangements are made. Patients with high deductible insurance plans will be required to pay 50% of the expected sleep services approved charges at the time the sleep study is scheduled, with the balance paid once the study has been completed.
What if I do not agree with the amount of the payment by my insurance carrier?
If you do not agree with, or understand the amount of the payment that has been made by your insurance carrier, you must contact your insurance carrier directly. It is your responsibility to contact your insurance carrier to receive an explanation of your benefits.
What if I am having trouble paying my account?
If you are having trouble paying your bill, please contact the Billing Department. Special arrangements can be made on an individual basis as needed, but it is important that you communicate with the Billing Department.
When is payment for services due?
Payment in full is expected for all services provided. Though the focus of the practice is patient care, payment for this care is necessary for the practice to remain viable. An individual's insurance will be billed for services provided, though co-pays are expected at the time of visit, or study.
Various payment methods are available, including cash, check, and credit card. The practice's billing department is available to handle insurance issues, and to answer questions that may arise regarding coverage of, or payment for, services rendered. Special arrangements for payment can be made on an individual basis as needed.
When should I expect to receive a billing statement?
If your insurance carrier is being billed for services rendered, you will receive a statement once your insurance carrier has made a payment. This statement will itemize any allowable charges that are not paid by your insurance carrier (i.e., co-pays, co-insurance and deductibles). Sleep Medicine Associates, PC can assist you in gathering information that you need to obtain any pre-approvals that your insurance carrier might require prior to services being rendered. However, it is your responsibility to understand your insurance benefits and what amounts your insurance will cover for services that are provided. Please contact your insurance carrier directly to obtain information concerning prior authorization requirements; what services your insurance will pay for, the level of payment and what your co-pays and deductibles might be.
If you are paying for services personally (out-of-pocket, without insurance) payment is expected at the time of service unless arrangements are made prior to service.
What does my statement mean?
The statement that you receive from Sleep Medicine Associates will itemize the allowable charges that are to be paid by you, after your insurance carrier has paid their portion of the fees for the services that have been provided. This amount is due upon receipt of the bill.

How do I know how much my insurance paid?
The amount paid by your insurance carrier will be recorded on the statement that you receive from Sleep Medicine Associates, PC. If you have questions concerning the amount that your insurance carrier paid for services rendered, you will need to contact your insurance carrier directly.
If I am having trouble paying my bill
If you are having trouble paying your bill, please contact the Billing Department. Special arrangements can be made on an individual basis as needed, but it is important that you communicate with the Billing Department.
What happens if I do not pay my bill?
If you do not pay your bill, you will receive a letter notifying you that you must contact the Billing Department to make payment arrangements. If payment arrangements are not made, your account will be turned over to a collection agency, (which could have an adverse affect on your credit rating), and you will be discharged from the practice, and not be able to receive further services.
